Agreement
- Massage therapy services. In accordance with all local laws and regulations, the Therapist will provide professional massage therapy and/or bodywork to the Company's employees. The Therapist holds a current, active license or registration, practices within the scope allowed where she is licensed, and upholds professional standards during all treatments.
- Term. This Agreement continues until the services are complete, on the end date above.
- Health information. The Therapist will gather appropriate health history, contact, and emergency information from everyone receiving massage. If someone refuses to provide it, the Therapist may refuse service to that person until it is given.
- Behavior. The Company will ensure, to the best of its ability, that everyone receiving massage behaves appropriately. If the Therapist judges a staff member's behavior inappropriate (including sexual innuendos or jokes, or unsuitable language, conversation, or gestures), she may end all treatments for that person, with full compensation paid whether or not the services were fully rendered.
- Fairness. Both parties agree to show no favoritism or discrimination regarding gender, color, age, sexual orientation, education, disability, veteran's status, religion, or national origin.
- Guarantee of service. If the Company does not allow the agreed services to take place, it remains responsible for the compensation above. If the Therapist cannot provide the agreed services, the Company owes no compensation for them.
